Many people have sent me email and mentioned that they wished my site was available in book format, because they wanted to have my site available to read while not at their computers, to give to their family members, and to give to members of their support groups who do not have internet access. So I have written the major portions of this site into book form. Many of the sections are just straight copy from my site pages with some modifications, other sections I wrote new.

Because I had to pay to have this printed into book form, and I will have to pay to mail it, I will have to charge you to purchase it.

How to purchase this book:

Price includes mailing by "media rate" [4th class] within continental United States. If you would like 1st class/priority mail, or if you are in Alaska or Hawaii, add $2. If you are in Canada, add $5. For all other countries, add $9. Please send email for information on mailing multiple copies to other countries. You are responsible for paying any customs/import duties which may be required.

California residents add 7.75% sales tax.

For orders of 10 or more books, please send email for pricing.

I can accept personal checks or money orders. International money orders must be in US funds with a US routing number. I will wait for personal checks to clear my bank before mailing. Books paid by money order will mail immediately. Please do not send cash.

The dimensions of the finished book are 5-1/2 x 8-1/2, or basically an 8-1/2 x 11 piece of paper folded in half. It is 364 pages long.
